What Makes a Patch a Patch

Two things separate a patch from embroidery worked straight onto a garment.
A finished edge
Because a patch is cut out, its border has to be sealed or it frays. On embroidered patches that is a dense satin stitch worked around the outline.
Enough body to hold its shape
A patch has to stay flat and stiff off the garment, which comes from the backing rather than the stitching.
Everything else — the design, the colours, the size — is the same problem as any other embroidery, and the basics of that are in what embroidery is.
What You Need
A base fabric
Twill is traditional; felt is easier for a first attempt because it does not fray while you work.
Cut-away stabiliser
This is the structural part. Tear-away is too weak for a patch, which has to survive being handled off the garment.
Thread
Embroidery thread for the design, and enough of your border colour, because satin borders eat far more thread than people expect.
A way to attach it
Iron-on adhesive backing, or the intention to sew it on.
Method 1: Machine-Embroider a Patch
The route that gives a traditional patch with the least hand skill.
Step 1: Set up the design with a border
The stitch file needs a satin-stitch border around the outline. Many patch designs are sold with one built in; if you are digitising, that border is not optional.
Step 2: Hoop fabric over cut-away stabiliser
Both layers, drum-taut. The stabiliser is what the patch will be made of structurally, so do not substitute tear-away here.
Step 3: Stitch a placement outline first
Run the outline stitch so you can see exactly where the patch edge will fall, then stitch the design inside it. This is what keeps the border even all the way round.
Step 4: Stitch the design, then the satin border last
Fills and details first, then the dense border over the top of the outline. Stitching the border first leaves the fill crossing it and looking rough.
Step 5: Cut out close to the border
Trim just outside the satin stitching with sharp scissors. Cutting into it unravels the edge you just built.
Step 6: Seal and back
A light pass of fray sealant on the cut edge, then iron-on adhesive backing if the patch is meant to be pressed rather than sewn.
Hoop size caps your patch size
A patch has to fit inside the hoop in one hooping, because splitting a design across two hoopings shows badly on something this small. A 4″ x 4″ hoop is fine for most patches; larger ones need a 5″ x 7″ machine.

Method 2: Hand-Embroider a Patch
Slower, but it needs no machine and no digitising, and felt makes it forgiving.
Step 1: Draw the outline on felt
Felt does not fray, so the edge is far more tolerant of imprecise cutting than twill would be.
Step 2: Hoop it and stitch the design

Step 3: Work a satin or blanket border
Satin stitch gives the classic sealed patch edge; blanket stitch is quicker and gives a more handmade look. Either seals the perimeter.
Step 4: Cut out and back it
Trim just outside the border, then either iron on adhesive backing or sew the patch on directly.
Method 3: Laser-Cut Leatherette Patches
Not embroidery at all, but it produces the same object and is worth knowing because it solves the two hardest parts: fine detail and edge finishing.

Detail is unlimited
Thread has a minimum practical stitch width, which is why small text and thin lines are hard to embroider. A beam has no such floor, so fine logos and small type reproduce cleanly.
No digitising
The artwork goes straight to the machine as vector or raster, with no stitch file in between.
The edge finishes itself
Cutting and sealing happen in the same operation, so there is no satin border to stitch and no fraying to prevent.
What you give up is texture. These patches are flat, and a hand can tell immediately that they are not thread.

A one-off digitising or setup fee, then a per-patch price that drops sharply with quantity. Minimum orders are common.
When it makes sense
Fifty or more identical patches. Below that, and especially for one-offs, making them yourself wins on both cost and turnaround.
How to Attach a Patch
Three options, and they differ mostly in how permanent they are.
Iron-on
Adhesive backing activated with heat. Fast and neat, and the bond is good but not indestructible on items that get heavy use.
Sewn on
The most durable answer, and the only sensible one for workwear or anything laundered industrially. Stitch around the very edge of the border.
Pressed then stitched
Press it to hold the position, then stitch around it. This is how commercial patch application is usually done, because it solves alignment and durability separately.

What Is the Best Way to Make Patches?
You want a traditional embroidered patch
Machine embroidery, with cut-away stabiliser and a satin border. A PE535 at $299.99 has enough hoop for patch-sized work.
Your logo has small text or thin lines
Laser-cut leatherette. Thread has a practical minimum width and small type simply will not resolve; a beam has no equivalent limit.
You want one patch as a gift
By hand on felt. Under thirty dollars of materials and no digitising, and the handmade look is the point rather than a compromise.
You need fifty or more
Order them. Per-patch prices fall steeply with quantity and the setup fee is spread across the run.
Conclusion
