This is a wooden checklist to make sure you children have what they need before going out the door. It is made out of 3mm plywood, card board. There are 3 layers of plywood and a layer of cardboard to add space for the answers to slide.

The Check list front also has the slides on that panel.
The middle piece and back pieces are cut from a 12" x 19" piece of 3mm unfinished Plywood. Once cut out they can be glued together.
The cardboard layer is a filler so the answer slide move freely. The setting I used were for a cereal box.

With the back and center piece glued together, wax the slide pieces and put the slides place. Then glue the cardboard filler and front on. I also included a file for a hanger and feet to glue on the back.
